Job Description: |
With our growing team of therapists, Sawtooth Mountain Clinic is looking for a compassionate, organized, and detail-oriented person to fill the brand-new role of Behavioral Health Care Coordinator.
The Behavioral Health Care Coordinator is responsible for supporting individuals with behavioral health needs by providing care coordination, resource navigation, and scheduling assistance. This role ensures timely access to services by managing provider schedules, coordinating client appointments, and assisting with patient follow-ups. The ideal candidate will have strong organizational skills, attention to detail, and the ability to work collaboratively with clients, healthcare professionals, and community organizations.
Primary Responsibilities: • Assess client needs and provide education about service options. • Coordinate behavioral health services, including referrals to therapists, psychiatrists, and community resources. • Monitor client progress and provide follow-up support to ensure continuity of care. • Advocate for clients by addressing barriers to care, such as transportation, financial concerns, childcare, food insecurity, and access to treatment. • Educate clients and families on behavioral health conditions, coping strategies, and available resources. • Maintain accurate documentation and case notes in accordance with legal and organizational requirements. • Participate in team meetings, case consultations, and training sessions to enhance professional knowledge and skills. • Manage provider schedules and appointment availability to optimize efficiency. • Schedule client appointments, follow-ups, and behavioral health services based on provider availability. • Ensure scheduling templates are updated and aligned with provider preferences and clinic operations. • Communicate appointment confirmations, reminders, and changes to clients and providers. • Coordinate rescheduling and cancellations while maintaining continuity of care. • Collaborate with administrative and clinical teams to improve scheduling processes.
