Qualifications and Requirements

o Associate’s degree (Required), Four-year in Human Services, Sociology or Psychology Preferred
o A valid driver’s license and daily access to reliable transportation to travel throughout the Twin Cities
o Experience working within the social service field (Minimum of 1 year required)
o Current vehicle insurance, and a clean driving record
o Successful clearance from a DHS background study
 
Job Description:
Our Outreach Specialists are responsible for providing direct, person-centered, services to individuals within the community. Working with the Person Served, Outreach Specialists focus on teaching and building skills that enhance a person’s ability to live independently.


Primary Responsibilities
o Provides Direct Service, using a person-centered approach to support individuals in reaching a higher level of independence in all areas of their life through independent living skills, behavioral management, and program services.
o Carry a caseload of 12-15 Individuals based within the Twin Cities metro area
o Meet with persons served in their homes or in the community on a weekly basis
o Monitor, document and evaluate goals for each person served
o Complete 245D annual documentation for the Person Served
