Job Description: |
Joseph Companies is seeking a part-time, 20-24/hrs, Community Liaison for our growing home healthcare organization. We are headquartered in Rochester, MN and currently have 23 locations throughout MN & WI. This position will be located at our Mankato, MN location. We are seeking a dynamic, high-energy, team player to join our team.
Position duties:
Networking and building relationships with hospitals, skilled nursing and other facilities to educate them on our various home health services. Maintain relationships with referral sources by providing routine interaction to continually learn what their needs are and how our organization can exceed them. Participates in patient care meetings/conferences as appropriate Facilitates communication with and between the organizational team and referral sources, clients/family members and others to ensure a positive relationship and compliance with governmental regulations and homecare requirements and protocols Participates in development of policies, procedures, goal setting and planning specific to meeting the needs of clients/families and caregivers. Prepare business plans and maintain target lists that will ensure that we are expanding our services in all designated areas. Qualifications:
Associates Degree in Marketing, Business, or a health-related science (e.g., nursing, pharmacy, etc.) Minimum of two years healthcare or related industry sales experience Knowledge of hospital, skilled nursing & other facilities, case management and discharge planning service needs Excellent organizational, interpersonal, presentation and communication skills Ability to travel within assigned territory for meetings & conferences Track record of success in growing census Strong existing relationships with referral sources in the market needed We offer a comprehensive benefits package & competitive pay.
