Bachelor's degree in social work and LSW, or a bachelor's degree in a related field such as psychology, human services, or sociology, or Registered Nurse, or Public Health Nurse Must have at least two years full-time social service experience in the education or treatment of persons from vulnerable populations Background clearance Ability to effectively communicate in English, written and verbally Driver's license, reliable vehicle you can use for work purposes, acceptable driving record, and proof of auto insurance Must be at least 18 years of age NICE TO HAVE:
Experience in CADI, BI, EW, CDCS, and AC Knowledge of MA, Medicare, and MN Healthcare programs Knowledge of community resources and providers Great organizational skills and attention to detail Knowledge of computer basics and familiarity with Microsoft Office suite Bi-lingual oral and written skills in Spanish, Hmong, Somali |

Thomas Allen's Case Management Division is Hiring!
LOCATION: Office in Edina, MN. Travel throughout the Twin Cities is required for meetings and visits with individuals served. Based on caseload, travel throughout other parts of Minnesota may be required (temporarily working remotely).
HOURS AVAILABLE: Flexible 8am - 4:30pm, Monday - Friday
JOB SUMMARY: As a Case Manager, you will coordinate assessments, create and review care plans, and coordinate services with individuals served and the interdisciplinary team. You will also connect individuals served with community resources and ensure waiver coordination and documents meet lead agency guidelines. This position will work with a combination of CDCS, CADI, BI, EW, or AC waivers. Each Case Manager is an advocate for the individuals they work with and helps to make a difference in their lives.
