Allina Health Home Care Services provides skilled medical care for patients and families needing rehabilitation, advanced-illness or end-of-life care. With headquarters located in St. Paul, Minn., services are provided in 30 Minnesota counties, 24 hours a day, seven days a week.
Responsibilities
Req ID: 66793
Provide safe competent cares to hospice patients to include bathing, skin care, assistance with bowel programs, simple wound cares, ROM and exercise programs and homemaking activities
Effective communication, which contributes to quality care and team cohesiveness
Documentation meets regulatory standards
Computer skills to document and email proficiently
Timely logging of hours to assure accurate billing
Assumes responsibility for meeting own education needs
Complete agency, state, and federal regulatory requirements for continuing education
Education
High school diploma or GED
NA-R/HHA certification
Current on the Minnesota CNA Registry
Qualification/Skills
Experience
At least 1 year of HHA experience is required, 3 years experience is preferred
Previous Home Care/Hospice experience preferred
Skills
Needs strong understanding of professional boundaries
Must be able to lift 45 lbs
Good understanding of body mechanics
Strong communication skills-written, verbal, listening with attention
Good problem solving skills
Good time management of self and others
Good computer skills
Able to function in an electronic medical record.
Licensure/Certifications
HHA Certification or 75 hours of comparable competency testing
Maintain ongoing requirements of 12 hours per calendar year of continuing education
Work Schedule
40 hours per week
Day shifts
Float position
Every 8th Weekend
