Job Description:
Mental Health Practitioner Care Coordinator- Crisis:

Do you have a passion for providing supports to those diagnosed with a mental health disorder or looking to gain experience? If so, Central Minnesota Mental Health Center (CMMHC) is a great place for you to begin or grow your career working with those diagnosed with a mental health disorder. We have opportunities to support a high volume of clients with complex diagnoses and co-occurring disorders as a Mental Health Practitioner or Mental Health Rehabilitation Worker in our Crisis program. CMMHC provides home, community and residential-based mental health services to individuals who are experiencing a mental health crisis in a four-county area (Stearns, Benton, Sherburne & Wright), with Crisis offices located in St. Cloud and Monticello. This position will be trained in our adult 5 bed residential unit. We will provide extensive training for qualified candidates.

We support programs on a 24/7 basis, this position would work:

Full-time morning shifts at our St. Cloud location with an every other weekend rotation

Our Mental Health Practitioners and Mental Health Rehabilitation Workers responsibilities include:

Provide residential treatment services including skills groups and one on one meeting with clients

Complete assessments and develops treatment plans

Coordinate services with other programs and providers

Consult and follow guidance from the Clinical Supervisor/Mental Health Professional

Required Qualifications:

Has at least 4,000 hours of supervised experience in the delivery of services to adults or children with

- mental illness, substance use disorder, or emotional disturbance; or

- Traumatic brain injury or developmental disabilities and completes training on mental illness, recovery from mental illness, mental health de-escalation techniques, co-occurring mental illness and substance abuse, and psychotropic medications and side effects;

Or

Holds a master's degree or other graduate degree in behavioral sciences or related fields

Or

Holds a bachelor's degree in behavioral sciences or related fields and completes a practicum or internship that (1) requires direct interaction with adults or children served, and (2) is focused on behavioral sciences or related fields

And

Clear DHS background study and sexual exploitations screening

Valid Driver's license and acceptable driving record.

Adult Targeted Case Management:

Our Targeted Case Management (TCM) team is expanding. Our program provides a passion driven, client-centered work environment with a unique blend of independence and team support.

Central MN Mental Health Center has opportunities for full time Adult Targeted Case Management positions. TCM provides home and community based mental health services to adults with serious and persistent mental illness (SPMI). The duties may include, but are not limited to:

Be familiar with the implementation of Health Integration, including care coordination.

Establish relationships with assigned clients and their supports, while addressing goals defined in their treatment plan.

Assess, refer, coordinate, and monitor services.

Communicate regularly with all involved providers involved to ensure appropriate care, reduce duplication of services and promote client autonomy.

Wage is dependent on experience and paid on an hourly basis.

This position has the potential to be located out of our St. Cloud, Waite Park, Monticello, Elk River or Buffalo office dependent on applicant preference.

Ability to work remotely temporarily due to Covid-19.

Qualifications

The Targeted Case Manager shall manager shall meet the following qualifications defined further in Minnesota Statute 245.462 Subdivision 4:

Bachelors degree in one of the behavioral sciences or related fields from an accredited college or university

2000 hours experience working with adults with serious mental illness and/or with children with severe emotional disturbances
