Mental Health Practitioner Care Coordinator- IRTS:
Do you have a passion for providing supports to those diagnosed with a mental health disorder or looking to gain experience? If so, Central Minnesota Mental Health Center (CMMHC) is a great place for you to begin or grow your career working with those diagnosed with a mental health disorder. We have opportunities to support clients with complex diagnoses and co-occurring disorders as a Mental Health Practitioner Care Coordinator in our Northway Intensive Residential Treatment Services (IRTS) program. CMMHC provides intensive residential treatment services to individuals who are experiencing significant difficulty with daily life activities as a result of a serious mental illness.
Since we support programs on a 24/7 basis, we have a variety of shifts and schedules available:
Full-time
Days with every other weekend involvement. This position requires scheduling flexibility that may include evenings and weekend shifts
Our Mental Health Practitioner Care Coordinator responsibilities include:
Provide medically necessary rehabilitation services and adult rehabilitative mental health services
Development of assessments and individual treatment plans with clients
Coordinate services with community providers/services
Provide group facilitation and participate in curriculum development
Leading review meetings
Required Qualifications:
Has at least 4,000 hours of supervised experience in the delivery of services to adults or children with
- mental illness, substance use disorder, or emotional disturbance; or
- Traumatic brain injury or developmental disabilities and completes training on mental illness, recovery from mental illness, mental health de-escalation techniques, co-occurring mental illness and substance abuse, and psychotropic medications and side effects;
Or
Holds a master's degree or other graduate degree in behavioral sciences or related fields
Or
Holds a bachelor's degree in behavioral sciences or related fields and completes a practicum or internship that (1) requires direct interaction with adults or children served, and (2) is focused on behavioral sciences or related fields
And
Clear DHS background study and sexual exploitations screening
Valid Driver's license and acceptable driving record.
Maintain First Aid and CPR Certification
Preferred Qualifications:
Experience and training in behavioral learning techniques, rehabilitation theory and practice
Experience with facilitation of educational groups

Clinical Trainee: Care Coordinator Team Lead:
Central MN Mental Health Center has an opportunity for a Full-Time Clinical Trainee: Care Coordinator Team Lead in our Northway Intensive Residential Treatment Services (IRTS). The Clinical Trainee provides intensive residential treatment services to recipients who are experiencing significant difficulty with daily life activities as a result of a serious mental illness in a comprehensive 24-hour service. The Clinical Trainee serves as the lead to the Mental Health Practitioners: Care Coordinators and works closely with the Program Director to ensure the clinical needs of the program are met. The Duties may include but are not limited to:
Conduct diagnostic and other required assessments, psychoeducation, family education in residential setting
Provide medically necessary rehabilitation services and adult rehabilitative mental health services
Offer individual therapeutic sessions to clients weekly
Direct and oversee the development of assessments and individual treatment plans with clients
Coordinate services with other programs and providers
Assist with coordinating appropriate client care and scheduling daily programming
Provide group facilitation and participate in curriculum development
Leading review meetings
Qualifications
Master's Degree in Psychology, Social Work, or Marriage and Family Therapy; Social Work degree Preferred
Licensed Mental Health Professional as defined in 9505.0371 Minnesota Rule, Subp 5A, or be a Master's Level Mental Health Practitioner working as a Clinical Trainee on track for licensure as a Mental Health Professional as defined in 9505.0371 Minnesota Rule, subp 5C; and
Clear DHS background study and sexual exploitations screening
Valid Driver's License and acceptable driving record.
