We are asking for information about you because some of the services you may receive are paid for by federal money, and we are required to track information to report to the federal government.

Data Privacy

Sensitive information you provide, such as your Social Security number (SSN), date of birth, gender, and race, is used only in an aggregate form to measure the performance of our service. This information is not released to employers. When requesting sensitive data online, we secure its delivery to the database with encryption software called Secure Sockets Layer. 

First Name Type in your first name of the primary user of the account.
Middle Initial Type in your middle initial, if any. (Capitalize)
Last Name Type in your last name of the primary user of the account.
Street Address Line 1 Type in your street mailing address.
Street Address Line 2 Type in additional mailing information such as an apartment number or post office box number.
City Type in your city.
State/Province Select your state or province from the drop-down menu. You can select a blank if you currently reside outside of the US or Canada.
Zip/Postal Code Type in your ZIP or postal code.
Phone Number Type in your phone number starting with the area code.
Alternate Phone Number Type in an alternate phone number, cell number or a number where a message can be left for you.
Email Address

Social Security Number

Enter your Social Security number (SSN) in the fields provided. Your SSN, and other sensitive data such as your date of birth, gender and race, is encrypted for transmission and stored in a secure database where it is not share with or accessible to anyone not authorized to receive the data. This information is used to measure the performance of our service. If you are strongly opposed to using your SSN, enter a number starting with a 9 followed by eight random numerials. Where Did You Hear About Us? Select an option from the drop-down menu.
Education From the drop down menu, select the highest level of education you have completed.
Are You In School? You are considered to be in school if you are attending school full time or are between terms and intend on returning to school.
Currently Employed?

You are considered to be working if you are currently doing any work as a paid employee (even if you are temporarily absent due to illness, bad weather, vacation, labor-management dispute or personal reasons), work in your own business, or work 15 hours per week or more for a family member. "Separation Notice" should be checked when a person who, although employed, either (a) has received notice of termination of employment or (b) is currently on military active duty and has been provided with a firm date of separation from military service.

Have you served in the U.S. Military?
  • An individual who served on active duty for more than 180 days with other than a dishonorable discharge, or discharged because of a service connected disability, or received a campaign badge and was discharged with other than a dishonorable discharge. An individual who is a current service member in active status (including separation leave) and within 24 months of retirement or 12 months of separation from service is considered a Transitioning Service Member and eligible for Veterans Employment Services.
  • The Minnesota Veterans Preference Act (VPA) grants most Veterans a limited preference over non-Veterans in hiring and promotion for most Minnesota public employment positions, as granted in Minnesota Statutes 197.48, 43A.11, and 197.455. The Minnesota VPA Statutes apply to Minnesota public employment, "civil service laws, charter provisions, ordinances, rules or regulations of a county, city, town, school district, or other municipality or political subdivision of this state."
  • For Minnesota VPA purposes, Minnesota Statute 197.447 provides the definition/eligibility requirements of a "Veteran." Minnesota Statute 197.46 grants most Veterans protection against unfair dismissals and demotions from most Minnesota public positions, without first providing the Veteran a VPA hearing. Spouses of Veterans are not included. Minnesota Statute 197.46 exempts the following public positions, "Nothing in section 197.455 or this section (197.46 shall be construed to apply to the position of private secretary, superintendent of schools, or one chief deputy of any elected official or head of a department, or to any person holding a strictly confidential relation to the appointing officer."
Others Eligible for Veterans Priority?

Persons are also eligible for veteran’s priority if they are/were the spouse of a veteran and can claim any of the following: the veteran died on active duty, or died of a service-connected disability, or has a total permanent service-connected disability, or is missing in action, or was a prisoner of war.

Migrant Worker or Seasonal Worker?

A person and his or her dependent(s) whose agricultural labor requires travel to a job site such that the person(s) are unable to return to a permanent place of residence within the same day; or a disadvantaged person and their dependent(s) who, for 12 consecutive months out of the 24 months prior to application for the program, has been primarily employed in agricultural labor that is characterized by chronic unemployment or underemployment.

Migrant Worker Status Choose the appropriate status from the drop-down menu: Migrant Farmworker, Migrant Food Processor, Seasonal Farmworker.
Separation Date from Active Duty

To change the entry, select the date of separation from the drop-down menus.

180 Days or More of Active Duty Indicate if you served 180 days or more of active duty for other than training purposes.
Are You a Campaign Veteran?

This is an eligible veteran who served on active duty in the U.S. armed forces during a war or in a campaign or expedition for which a campaign badge or expeditionary medal has been authorized as identified and listed by the Office of Personnel Management (OPM).

Service-connected Disability Select the appropriate answer if you are a service-connected disabled veteran and, if so, whether your rating is for less than 30% or for 30% or more.
Currently or at risk of being homeless Select the check box if you have no current permanent residence or are at imminent risk of being homeless.
Username

The username is not case sensitive. You may use special characters and numbers. Enter at least 6 characters.

Password Username and password cannot be the same. Enter a password of 6-12 characters. Password is case sensitive.
Secret Question Enter a question with a one-word answer that only you will know. Field limit is 100 characters.
Answer Enter the one-word answer. The answer cannot contain spaces and is limited to 20 characters.
Birth Date Select your date of birth (month, day and year) from the drop-down menus provided.
Gender Indicate your gender by selecting one of the options: Male or Female.
Ethnicity: Hispanic/Latino? Select YES or NO to the question: Are you of Cuban, Mexican, Puerto Rican, South or Central American, or other Spanish culture in origin, regardless of race?
Race From the list provided, choose all racial origins that apply to you.
Persons With a Disability Indicate by selecting from the drop-down menu whether you have a physical or mental impairment that substantially limits one or more of the major life activities.
Next Once you have completed all required fields, select “Next.”

Transition Assistance Program

The Transition Assistance Program is a series of workshops to meet the needs of separating service members during their period of transition into civilian life by offering job-search assistance and related services such as resume development or interview techniques. TAP is only tracked for three years after attendance.
